This program is based on the ASK-20 questionnaire to target barriers to adherence and individualized the educational session to the patient. Methods and Results : The program has been entirely developed based on the guidelines from the “Haute Autorité de Santé” (HAS). Each barrier identified with the ASK-20 questionnaire is linked to specific actions to individualize the therapeutic education consultation, engage the patient and improve adherence with different tools created. Three types of tools were selected and created : (1) A therapeutic planning to talk with the patient about his everyday life with medicines. (2) Brochures about diabetes and heart failure to approach more knowledge about the disease and medication. (3) Pillboxes to assess the patient’s ability to use it. Other non-educational actions could be used such as medication improvements through pharmaceutical interventions with geriatricians. Once the program created, a 2 half-day training session about therapeutic education was given to 2 clinical pharmacists, 6 postgrad students and 1 pregrad student in Pharmacy. They took part in a theoretical training session involving brainstorming and role-playing. The global satisfaction towards the training reached 9,1/10. Finally, an evaluation of the program was carried out with 2 patients-as-partner and 2 pilot patients. Satisfaction of therapeutic education consultations for pilot patients reached 9/10. Conclusion : This qualitative study highlighted the importance of creating a structured TPE program.
