Abstract
Introduction. Chronic Kidney Disease (CKD) is an irreversible and silent disease. Pharmacists could play a major role by providing individual therapeutic education sessions focusing on nephroprotective measures and medications during the patient’s hospitalization. Objectives. To evaluate the benefit of providing educational sessions for patients with CKD, several parameters were assessed. First, the patient’s progress in understanding his disease, both in terms of nephroprotective measures and drug treatment. In addition, the feasibility of integrating these sessions into daily practice of a clinical pharmacist will be determined. We will evaluate time required for the sessions, the patient’s perception of the pharmacist’s role, and the patient’s opinion regarging the sessions being in-hospital. Finally, patient satisfaction regarding these sessions will be assessed. Method. An interventional study was conducted in the CHwapi hospital wards over a 9-week period. Three therapeutic education sessions were developed by a clinical pharmacist and validated by 4 nephrologists, 3 pharmacists, and 2 expert patients. Results. Forty-three patients with CKD were enrolled. The results suggest an overall improvement of 35.37 % in nephroprotective measures and an increase of 22 % in knowledge of drug indications in patients taking ≥ 10 drugs (N=24). On average, the total time spent per patient was 2h08. Most patients (N=26) strongly agreed that pharmacists should provide these sessions, and 14 agreed. Twenty-eight patients totally agreed that these sessions should take place during hospitalisation, and 14 agreed. Finally, 51.2 % (N=22) and 46.5 % (N=20) of patients are very satisfied and satisfied, respectively, with these sessions. Conclusion. This study highlights the advantage of hospitalization to emphasize the fundamental concepts of nephroprotection. Thanks to the highly appreciated therapeutic education sessions, the clinical pharmacist makes the patient aware of his role as a participant in his treatment and the importance of incorporating these measures into his daily life. The pharmacist plays a crucial role in improving knowledge about CKD and medications.
