- Polycystic ovary syndrome (PCOS) is the leading cause of female infertility worldwide. As infertility is a major public health problem, there is a need to improve the management of patients with PCOS in order to optimise pregnancy rates. Treatments used in the context of assisted reproduction are already well established in clinical practice. Nevertheless, therapeutic failures and the occurrence of complications are not negligible. For this reason, researchers have been looking at other alternatives that may have a positive effect on ovulation induction in order to expand the therapeutic possibilities for women with PCOS. This thesis will first present all the information necessary for a good understanding of PCOS, namely: diagnostic criteria, pathophysiology, symptomatology, and obstetric risks and complications. Then, the different treatments to treat infertility will be developed with a particular focus on inositol, a vitamin supplement, as well as on tamoxifen and letrozole which do not have a marketing authorisation for this indication. Finally, a study demonstrating the superiority of letrozole over clomiphene citrate will be detailed to reinforce the arguments made. A conclusion of the key ideas regarding the optimal management of infertility in women with PCOS will conclude this paper.
