Abstract

enType 2 diabetes (T2D) primarily affects older adults, particularly patients aged 75 and over, in whom therapeutic management is complicated by frailty, polypharmacy, and age-related physiological changes. Oral semaglutide, a recently available GLP-1 analogue, offers an interesting alternative to injectable forms and may improve treatment acceptability. This thesis aims to assess the role of oral semaglutide in managing T2D in patients aged 75 and over, focusing on its efficacy and safety. A literature review was conducted based on clinical trials, real-world observational studies, and international guidelines. Available data show a significant reduction in HbA1c and moderate weight loss in patients aged 75 and over. The tolerability profile is generally favorable, although gastrointestinal adverse effects and treatment discontinuations are more common in this population. The risk of hypoglycemia remains low except when combined with other glucose-lowering therapies. Cautious use of the medication is advisable in clinical practice in patients aged 75 and over given the limited data available in the literature.
