Abstract

enVitiligo is an autoimmune skin disease characterized by progressive depigmentation resulting from the destruction of melanocytes, affecting approximately 1% of the global population. Beyond its clinical manifestations, this condition has a significant psychosocial impact, justifying the need for a comprehensive approach to patient care. The aim of this thesis is to provide an overview of current knowledge on vitiligo, addressing its pathophysiology, existing therapeutic options, and recent advances arising from an improved understanding of the immune mechanisms involving the IFN-γ/JAK-STAT pathway. Particular emphasis is placed on topical ruxolitinib (Opzelura®), the first approved treatment specifically targeting this pathway in non-segmental vitiligo. The analysis of the main phase III clinical trials demonstrates clinically meaningful repigmentation outcomes, associated with a generally favorable safety profile. Finally, this work highlights future research perspectives, including the evaluation of long-term efficacy, relapse prevention, the potential role of combination therapies, and the exploration of new immunological targets involved in disease chronicity.
