- Migraine is a chronic and highly disabling neurological disorder, ranking among the leading causes of disability worldwide. Recent therapeutic advances, particularly the development of treatments targeting the calcitonin gene-related peptide (CGRP), have profoundly modified the management of this condition. However, a non-negligible proportion of patients remains insufficiently relieved, suggesting the involvement of complementary pathophysiological pathways. In this context, the pituitary adenylate cyclase-activating polypeptide (PACAP) has emerged as a major mediator of the trigemino-vascular system. Experimental data show that the administration of PACAP, notably in the form of PACAP-38, induces meningeal vasodilation and may trigger migraine attacks, independently of the CGRP pathway. These observations have led to the development of therapeutic strategies targeting PACAP or its receptors. Early clinical trials suggest that direct ligand blockade could represent a promising approach, particularly in patients who are non-responders to anti-CGRP treatments, although further studies are required to better define its efficacy and therapeutic positioning.
