Abstract
In elective orthopaedic surgery, perioperative blood loss is still an elementary problem. For a long time, allogeneic blood transfusion remained the conventional treatment to reduce haemoglobin levels. However, it has since been shown that this procedure is associated with numerous risks. In response to this problem, patient blood management programmes have been developed, including the use of tranexamic acid. Tranexamic acid is an antifibrinolytic agent used in the perioperative period to restrict blood loss. Since the first studies on its value in orthopaedic surgery in the 1990s, research has continued to evolve. This thesis has two objectives. The first is to determine the theoretical and practical place of tranexamic acid in total knee arthroplasty, hip arthroplasty, unicompartmental knee arthroplasty and their respective revision. The second is to investigate the optimal method of administration in each of these procedures and the methods used in clinical practice. To address this, this brief will first cover the generalities of tranexamic acid. Then the goal-oriented parts will be developed using various studies and guidelines and an anonymous survey of orthopaedic surgery specialists and physician assistants. The concordance between theory and practice will also be discussed. Finally, the conclusion will focus on the results obtained, their discrepancies and their impact on practice and future research.
