Abstract
Clinical decision support systems (CDSS) inserted into computerized physician order entry reduce medical error during prescription. Relevance of medication-related alerts is essential to prevent useless interruptions, alert fatigue and bypass. Objectives : Optimization of CDSS at order entry in pregnant and breastfeeding women in a teaching hospital using Thériaque® as knowledge base for CDSS. The overall study design is based on the PDCA method. Plan: classification of the drugs involved in 80% of bypassed pregnancy/ lactation alerts at order entry into optimization based on literature of (a) display process, (b) severity level, and (c) content to suggest an alternative. Do: implementation of optimizations in the system. Check: evaluation of the effect of these 3 simultaneous optimization strategies on the pregnancy/ lactation alert rate and overall pregnancy/ lactation alert bypass rate. Act: record results and any discrepancies and suggest improvements.
