- Abstract
- High blood pressure is a major public health problem. Over 1 billion people are affected. Several treatments are available, but none has allowed optimal control of blood pressure in most patients due to limitations such as patient non-compliance and complex treatment regimens. In this context, oligonucleotides, including interferent RNAs (siRNA), represent an innovative therapeutic approach. Zilebesiran, a siRNA specifically targeting the angiotensinogen, promises prolonged suppression of the renin-angiotensin-aldosterone system with biannual administration that could solve the compliance problem. This work explores the mechanisms of action of oligonucleotides, the development of zilebesiran and its clinical results. Phase 1(NCT03934307) KARDIA-1 and KARDIA-2 studies showed significant and prolonged reductions in blood pressure, with a favourable tolerance. However, these studies have limitations, including short follow-up time but also lack of direct comparison with standard treatments, so it is difficult to draw conclusions from these studies. Zilebesiran offers unique benefits, including simplified treatment and specific targeting, but its adoption requires long-term studies to confirm its safety and efficacy. This paper concludes that, although promising, zilebesiran will need to prove its clinical and economic added value to revolutionize HTA management.
