Abstract
Introduction: HABIT-ILE is an intensive therapy that focusses on training bimanual coordination with constant and concomitant stimulation of postural control and lower limbs. It has shown its effectiveness in stroke patients, but its implementation in clinical settings remains challenging. Aim: To evaluate the feasibility and effectiveness of a 2-week intensive HABIT ILE@home program compared with its on-site equivalent, maintaining an equivalent duration of practice. Methods: Adults with chronic stroke were randomized to two HABIT-ILE@home and HABIT-ILE@site groups. The home group followed the principles of site and was delivered by caregivers with remote monitoring by trained therapists. The use of a REAtouch®Lite remote rehabilitation device was set up to facilitate instruction delivery and remote monitoring. Two external graders were trained to rate assessments after they were recorded by the examiners. Results: Seventeen patients were randomized, nine and eight respectively in the home and site groups. The groups showed improvements in the 6MWT, TUG, MBT, COPM_P&S, SIS, ABILHAND and BBT NH scores. Conclusion: Our results confirm the study hypothesis that HABIT-ILE therapy can be implemented at home in stroke patients and appears to be comparable to on-site HABIT-ILE therapy in terms of improvement in motor function, activity limitation and participation. However, the sample size designed for this study is too small to demonstrate clear observations.
